Foundations San Francisco is seeking a dynamic and talented Intake Coordinator
Located in the heart of the Embarcadero district, Foundations San Francisco offers comprehensive, integrated outpatient treatment in a convenient urban setting. Flexible daytime and evening programs address addiction and any co-occurring mental health issues simultaneously in a supportive atmosphere. Patients are treated with dignity and respect as the staff focuses on each individual's unique needs.
The Intake Coordinator initiates and completes the referral and intake process, determines appropriate level of care and maintains follow-up communication with referral sources, families, patients and program staff to maintain a quality intake.
Key Responsibilities include:
Exhibits excellent customer relation skills as evidenced by supportive and constructive communication with all contacts including coworkers, patients, residents, visitors, families and referral sources.
Exercise good judgment, apply common sense and successfully handle ambiguity.
Understanding of diverse cultures and gender specific issues and the ability to incorporate needs of gender and culturally diverse groups into the organization.
Must have strong knowledge of a variety of computer software applications in word processing, spreadsheets, and presentations (MS Word, Excel, and PowerPoint).Proficiency in MS Access (or other database program) is preferred.
Knowledge of addiction, addictive process and recovery.
